Required Annual Update Forms

Use credentials provided by your student's school to login to Skyward Family Access. If you need help, please contact your student's school office or District Office (509) 754-2474.

1. Review the "General Information" that Ephrata Schools has on-file for your student. No edits are made to contact information on this screen. Any edits needed will be completed in step 1b.
2. Scroll down the page and complete the questions under the headline "Allow Publication of Student's Name for." By saying "Yes" to each of these sections, you authorize student information to be released to the following:

• Military Use: Student information can be sent to military recruiters. 
• Higher Education Use: Student information can be sent to institutions of higher education.
• Public Use: Student information (student of the month, team rosters, etc.) may be used on the Ephrata School District website and social media accounts and may be shared with news media.
• District Use: Student information may be used in school yearbooks, sports publications, photos, etc.

3. When finished, click "Complete and move to Step 1b" to advance.

If changes to address are needed, this is the screen in which to make these changes.

1. Update address information as needed
2. Click "Complete Step 1b and move to step 1c" to continue the Annual Update Forms

1. On this screen, you can verify your "Home Language." The Ephrata School District will communicate with you based on the information added here. If you speak English at home, messages will be sent in English. If you speak Spanish at home, messages will be sent in Spanish.
2. Check to make sure guardian information is correct for your child and make any needed edits.
3. Click "Complete Step 1c and move to Step 1d" to move on.

On this screen you can review and edit the names and contact information of people you'd like the Ephrata School District to contact in the event of an emergency at school. Primary guardians will always be contacted first. Emergency contacts are necessary in the event the primary guardian cannot be reached.

1. Make any needed revisions / updates. Note: Only three contacts are allowed.
2. Click "Complete Step 1d and move to Step 2" to advance.

In this section, you will be able to grant permission and acknowledge various policy statements from the Ephrata School District.

1. Click on the drop down box following each statement.
2. Confirm, deny or acknowledge as prompted.
3. Click "Complete Step 2 and move to Step 3" to advance

You've completed your annual updates! Simply click the "Submit Annual Student Update" button to send your completed forms to the Ephrata School District.
