Community Use of Outdoor Facilities
Currently, all indoor Ephrata School District Facilities are closed to public use until further notice as a precautionary measure to help mitigate social contact and minimize the spread of the COVID-19 virus.

  Community Group COVID-19 Facility Use Guidelines & Agreement

The following guidelines are from the Department of Health and Governor’s Office and must be followed for community groups to use Ephrata School District Facilities.  This is for OUTDOOR Ephrata School District facilities only at this time, as gyms remain CLOSED.


Please complete the following and return to the district office:


  1. Department of Health/WIAA COVID-19 Sporting Activities Guidelines:
    • Read and follow the current COVID-19 safety requirements

          Masks for everyone, wash hands, physical distancing, wellness screenings of participants, no tournaments at phase 1, 2 or 3, etc. (as of 3.12.21)

          As these guidelines are updated, teams must stay current and follow.

  1. Ephrata School District Facility Use Request Form and be on the district facility calendar
  1. A valid certificate of insurance, naming the Ephrata School District as ‘Additional Insured’
  2. Individually signed Release and Waiver of Liability Agreement for each player, coach, and volunteer attending practices/games
  3. Submit this completed form with the rest of the materials to Cathy Wulf at the district office.
  4. Meet with the district athletic director or administrator prior to first practice.
